Mayor Vilma Caluag personally supervised the enrollment of free hemodialysis program of the City Government of San Fernando as expected almost hundred of patients gathered in Heroes Hall on Thursday, September 29,2022.
The enrollment from morning session are patients from Barangay Calulut, Barangay Malpitic and Northville while afternoon session that will be accommodated were from Barangay Bulaon Resettlement, Barangay Malino and Barangay Panipuan.
Further, the City Health Office disclosed that it will hold another enrollment day for those who were not able to enroll during the previous sessions.
The patients are advised to bring the following photocopied requirements: 1. Medical Certificate (old or recent) 2. Medical Prescription for medicines being taken (reseta) 3. Valid ID or barangay certification/clearance/indigency that indicates the residence in the City of San Fernando. (CSF CIO)
